Rationale for Respiratory Home Care Within A
Health-Care Reform Environment

1A. In the contexts of health care reform, Kenneth Wyka talks about the radical changes in health care and their impact on health care in general, but specifically on the respiratory-care profession.  List and describe three major reasons for health-care reform, and at least three management models that have been used in an attempt to curb the escalating health-cost.

1B. According to Kenneth Wyka, the advent of health-care reform have significantly shift the delivery of respiratory care form the hospital to alternate sites like subacute and extended-care facilities, specialty centers and the home. This shift in delivery of care has changed attitudes and concepts regarding how continuing respiratory care can be provided. Subacute care, home care and pulmonary rehabilitation (PR) help make up the entity known as continuing care. (1) Review a web site or article dealing with these issues and post your views or perspective on how health-care reform have or will impact the respiratory profession in the current state of health care management.  (2). Define the current definition of pulmonary rehabilitation. (3). State the major goal of pulmonary rehabilitation along with its two principal objectives.

4A. Typically appropriate patient selection is the fist part of the pulmonary rehabilitation process. The next step in the process involves designing and implementing a program which uniquely addresses each patient’s specific condition, life situation and overall needs.  These needs may be grouped on the basis of being physiological, psychological, social and financial. When designing and implementing a PR program to meet the needs of each patient, essential components must be followed: (1). Identify five key components of a PR program. (2). Differentiate between closed-format PR programs and open-ended programs.

While program design and resources such as equipment and staffing are relatively straightforward, the ability to direct and motivate patients is more difficult but equally important to the overall development and success of a program.
